Interesting initiative. One thing I'd be curious about is whether you're also capturing failures that happen after the business is contacted. In my experience, agents often get blocked not because they can't reach a service, but because they end up in retry loops, hit unexpected tool failures, or lose context midway through a workflow.
I've been exploring similar runtime reliability problems through https://github.com/FailproofAI/failproofai. It focuses on execution guardrails rather than access itself, and I think the combination of understanding external bottlenecks and internal failure modes would paint a much more complete picture.
