Report missing coordinators in cluster health metric

This PR adds a new FactorCoordinatorReachability factor for cluster health, reporting the INTERVENTION_REQUIRED health level if not all coordinators are reachable from the cluster controller. Code probes and a new /fdbserver/clustercontroller/ClusterHealthMonitor/CoordinatorReachabilityFactor unit test are added, and documentation is updated.
